Step 2: Complete and submit the Online Program Information Request (if not already submitted)
Step 3: Once admitted to your online program, you are ready to contact the appropriate academic department for program advising.
Step 4: Florida state law and UWF Student Health Policy requires that All students comply with the 1) Measles, Mumps, Rubella (MMR), and 2) Meningitis/Hepatitis B immunization requirements Before registering for academic courses. An "Immunization Hold" is automatically placed on new student records which prohibit course registration until proof of immunization or immunization waiver forms(below) are received and processed.
Good News:
New students registering for Only online courses and who will not be physically present in any UWF classroom or on any UWF campus may 1) download, 2) complete, and 3) return Both the Measles, Mumps, Rubella (MMR) and Meningitis/Hepatitis B waiver forms below. Once both completed waiver forms are submitted and processed, the "Immunization Hold" will be removed and new students can proceed with course registration.
Waiver Forms
The two completed forms must be signed and returned to Student Health Services via email (immunizations@uwf.edu), via fax (850-857-6100), or by mail to: Student Health Services, 11000 University Parkway, Bldg 960, Pensacola, FL 32514. If mailing documentation, you must include your name, date of birth, and UWF ID number.
For more information on Immunization requirements and waiver information, students should visit the Student Health Services website at: http://www.uwf.edu/healthcenter/ImmRequirements.cfm
Step 5: When advising has been completed you are ready to register for coursework using your online student MyUWF account.
